{"id":11773,"date":"2026-02-05T12:17:00","date_gmt":"2026-02-05T11:17:00","guid":{"rendered":"https:\/\/sedanoc.rwdesarrollos2.es\/actualidad\/airef-warns-of-a-structural-deficiency-in-medical-leave-management-after-spending-triples-in-10-years\/"},"modified":"2026-02-05T12:17:00","modified_gmt":"2026-02-05T11:17:00","slug":"airef-warns-of-a-structural-deficiency-in-medical-leave-management-after-spending-triples-in-10-years","status":"publish","type":"post","link":"https:\/\/sedanoc.rwdesarrollos2.es\/en\/actualidad\/airef-warns-of-a-structural-deficiency-in-medical-leave-management-after-spending-triples-in-10-years\/","title":{"rendered":"AIReF WARNS OF A STRUCTURAL DEFICIENCY IN MEDICAL LEAVE MANAGEMENT AFTER SPENDING TRIPLES IN 10 YEARS"},"content":{"rendered":"<ul>\n<li><b><i>The Independent Authority for Fiscal Responsibility (AIReF) has detected a &ldquo;structural deficiency&rdquo; in the management of temporary disability, in a context where a near 60% increase in the incidence of common contingency medical leave was recorded between 2017 and 2024, alongside a 15% increase in average duration, causing expenditure on temporary disability to triple over the last ten years (since 2014)<\/i><\/b><\/li>\n<\/ul>\n<ul>\n<li><b><i>The independent body led by Cristina Herrero published this Wednesday a study on temporary disability (TD), which is part of three studies commissioned to the institution within the framework of the &lsquo;Spending Review 2022-2026&rsquo;<\/i><\/b><\/li>\n<\/ul>\n<p>Temporary disability &ndash;which provides temporary economic protection to workers who have suffered an illness or an accident&ndash; represents one of the main benefits of the Spanish welfare system and is the second largest spending component of Social Security, with an outlay of 16.5 billion euros in 2024, surpassed only by pensions.<\/p>\n<p><b>AIReF&#8217;s evaluation identifies a structural deficiency in the management of temporary disability, derived from the separation between the authority that grants the benefit (primary care physicians) and the financial responsibility for it (the National Social Security Institute), an issue that is aggravated by the decentralization of the Spanish healthcare system.<\/b><\/p>\n<p>This structural diagnosis also occurs in a context of deteriorating main indicators of temporary disability, with a near 60% increase in the incidence of leave for common contingencies between 2017 and 2024 and a 15% increase in average duration, especially in the highest-spending pathologies (mental health and musculoskeletal conditions), amid longer waiting lists in the healthcare system.<\/p>\n<h3><b>LEAVE FOR MENTAL ILLNESSES PRESENTS THE LONGEST DURATIONS<\/b><\/h3>\n<p><b>In a press conference, the president of AIReF, Cristina Herrero, explained that there has been an increase in common contingency episodes, rising from 4.7 million in 2017 to nearly 8.6 million in 2024, compared to the stability of professional contingencies (700,000 episodes per year).<\/b><\/p>\n<p>More specifically, AIReF has calculated that the incidence of common contingencies has increased from 21.4 cases per 1,000 affiliates in 2017 to 33.9 in 2024, while the average duration has grown from 40 days in 2017 to 45.9 days in 2024.<\/p>\n<p><b>According to Cristina Herrero, musculoskeletal and respiratory diseases have the highest levels of incidence, but the largest cumulative growth is seen in infectious and mental illnesses. Regarding the latter (mental illnesses), AIReF explained that they present the longest average durations, with an increase from 67 days in 2017 to 98.5 days in 2024.<\/b><\/p>\n<h3><b>TRANSITION FROM TEMPORARY TO PERMANENT CONTRACTS INCREASES THE PROBABILITY OF LEAVE<\/b><\/h3>\n<p>Furthermore, AIReF notes the influence of the evolving regulatory framework toward a progressively more protective approach since 2018, which has reversed previous restrictions and expanded economic protection during temporary disability episodes.<\/p>\n<p><b>Specifically, it highlights the restoration of the 100% salary supplement for public employees on temporary disability, which increased the probability of starting a medical leave by 40%.<\/b><\/p>\n<p>Meanwhile, the transition from temporary contracts to permanent contracts driven by the labor reform increased the probability of starting a temporary disability episode by around 30%.<\/p>\n<p><b>AIReF has also found, based on models and quantitative analyses, that 25% of individuals account for 55% of temporary disability episodes, indicating a phenomenon of recurrence.
